1. SPONSOR

The Imagine Initiative Korn Guitar Sweepstakes (the "Sweepstakes") is sponsored by Orchard Church, a Colorado nonprofit religious corporation and 501(c)(3) tax-exempt organization, located at 12405 E 120th Ave, Brighton, CO 80640 (the "Sponsor"). EIN: 35-2258509.

2. ELIGIBILITY

The Sweepstakes is open only to legal residents of the forty-eight (48) contiguous United States (excluding Alaska and Hawaii), who are at least eighteen (18) years of age at the time of entry. Residents of Alaska, Hawaii, Puerto Rico, the U.S. Virgin Islands, Guam, American Samoa, the Northern Mariana Islands, and all international locations are NOT eligible to enter or win.

The following persons are NOT eligible to participate:

●   Employees, officers, directors, and agents of Orchard Church

●   Members of the immediate family (spouse, parent, sibling, child) or household of any ineligible person

●   Any person who has won a prize from Orchard Church within the past twelve (12) months

●   Any person residing in a jurisdiction where the Sweepstakes is prohibited by applicable law

3. SWEEPSTAKES PERIOD

The Sweepstakes begins on September 18, 2026 at 7:00 PM (MST) and ends on October 16, 2026 at 11:59 PM (MST) (the "Sweepstakes Period"). The Sponsor's computer is the official timekeeping device. Entries received before or after the Sweepstakes Period will be void.

4. HOW TO ENTER

There are two (2) methods of entry. Entries from both methods are combined into a single drawing pool. Both methods provide equal odds per entry.

METHOD 1 — DONATION ENTRY (Online):

During the Sweepstakes Period, visit the Sponsor's official sweepstakes donation page at https://www.zeffy.com/en-US/donation-form/win-a-korn-band-signed-guitar-all-5-members-imagine-initiative-sweepstakes (the "Entry Page") and make a charitable donation to the Imagine Initiative in one of the following amounts: 

Donations are processed through Zeffy, a third-party nonprofit fundraising platform. Internet access and a valid payment method are required for online entry.

METHOD 2 — FREE MAIL-IN ENTRY (Alternate Method of Entry / "AMOE"):

NO PURCHASE OR DONATION IS NECESSARY TO ENTER OR WIN. To obtain one (1) free entry, handwrite your full legal name, complete mailing address (street, city, state, zip code), and phone number on a 3" x 5" card or piece of paper and mail it in a sealed envelope with sufficient postage to:

Mail-in entries must be received by October 16, 2026 at 11:59 PM (MST) to be eligible. Limit one (1) free mail-in entry per person per outer mailing envelope. Sponsor is not responsible for lost, late, misdirected, illegible, or postage-due mail.

5. ENTRY LIMITATIONS

Limit: One (1) free AMOE entry per person per outer mailing envelope. Paid donation entries are limited to a maximum of seven (7) entries per transaction. Total Entry Cap: 4,000 paid donation entries maximum. Free AMOE mail-in entries continue to be accepted through the end of the Sweepstakes Period regardless of whether the paid entry cap has been reached.

6. PRIZE

One (1) Grand Prize: One (1) brand new electric guitar signed by all five (5) members of Korn — Jonathan Davis, Brian "Head" Welch, James "Munky" Shaffer, Ray Luzier, and Ra Diaz (the "Prize Guitar"). The guitar was signed prior to the commencement of the Sweepstakes. Approximate Retail Value ("ARV"): $900.

The Prize Guitar and certificate of authenticity will be delivered via insured, trackable shipping carrier at Sponsor's expense to a valid address within the contiguous 48 United States. The prize is awarded "as-is" with no warranty, express or implied. No substitution, transfer, or cash equivalent is permitted except at Sponsor's sole discretion.

7. ODDS OF WINNING

Odds of winning depend on the total number of eligible entries received from both entry methods during the Sweepstakes Period. If the maximum of 4,000 paid entries are received and no AMOE entries are submitted, the odds of winning with a single entry are approximately 1 in 4,000. Actual odds will be determined at the close of the Sweepstakes Period.

8. DRAWING AND WINNER SELECTION

On October 22, 2026 (the "Drawing Date"), the Sponsor will conduct a random drawing from among all eligible entries received during the Sweepstakes Period. The drawing will be conducted by or under the supervision of Candi Barrow, the Director of Finance & Human Resources at Orchard Church, 12405 E 120th Ave, Brighton, CO 80640.

Prior to the drawing, the Sponsor will combine all eligible entries from both methods into a single master entry list with sequential entry numbers assigned. A random number will be generated using a certified random number generator (random.org or equivalent) displayed live. The entrant corresponding to the selected entry number will be the potential Grand Prize Winner, subject to verification.

9. WINNER NOTIFICATION AND VERIFICATION

The potential winner will be notified by phone, text, and/or email within 10 business days following the drawing and must respond within 30 days providing: (a) proof of identity, (b) proof of eligibility, (c) a completed and signed Affidavit of Eligibility, Liability Release, and (where permitted) Publicity Release. Failure to respond or comply will result in forfeiture and selection of an alternate winner.

Winner must provide a valid mailing address within the contiguous 48 United States. The Prize Guitar will be shipped within 14 days of winner verification.

10. TAXES AND FINANCIAL OBLIGATIONS

All federal, state, and local taxes on the prize are the sole responsibility of the winner. The winner will receive an IRS Form 1099-MISC since the prize fair market value exceeds $600. By accepting the prize, the winner consents to use of their information for tax reporting purposes. Winning is a taxable event; the winner is encouraged to consult a tax professional.

11. CHARITABLE DONATION — TAX DEDUCTIBILITY

Orchard Church is a tax-exempt organization under Section 501(c)(3) of the Internal Revenue Code. EIN: 35-2258509. Donations made through Method 1 may be tax-deductible as charitable contributions to the extent permitted by applicable law. Because sweepstakes entries are available at no cost through the AMOE, the sweepstakes entries carry no fair market value that would reduce the deductibility of the donation. A donation confirmation letter will be provided via Zeffy's automated system upon completion of a donation.

12. GENERAL CONDITIONS

By entering, each entrant agrees to be bound by these Official Rules and the decisions of the Sponsor, which are final and binding. Sponsor reserves the right to cancel, modify, or suspend the Sweepstakes at any time for any reason. Sponsor is not responsible for lost, late, or incomplete entries; computer hardware or software failures; failed transmissions; or errors in administration of the Sweepstakes.

13. RELEASE AND LIMITATION OF LIABILITY

By participating, each entrant agrees to release and hold harmless Orchard Church, its officers, directors, employees, volunteers, agents, and representatives from and against any claims, damages, losses, costs, and expenses arising out of participation in the Sweepstakes, acceptance or use of any prize, or any breach of these Official Rules.

14. PUBLICITY RIGHTS

By accepting the prize, the winner grants Sponsor permission to use the winner's name, city and state of residence, photograph, likeness, and statements for advertising and promotional purposes in any media, without further compensation, except where prohibited by law.

15. PRIVACY

Personal information collected in connection with this Sweepstakes will be used solely for administering the Sweepstakes and fulfilling the prize. Orchard Church will not sell personal information to third parties except as required by law or as necessary to fulfill the prize.

16. DISPUTES AND GOVERNING LAW

These Official Rules will be governed by the laws of the State of Colorado, without regard to conflict of law provisions. Any disputes will be resolved individually, without resort to class action, exclusively in the state or federal courts located in Adams County, Colorado.

17. SPECIAL NOTICE FOR RESIDENTS OF NEW YORK AND FLORIDA

RESIDENTS OF NEW YORK AND FLORIDA: The Sweepstakes may be subject to registration and bonding requirements under the laws of New York and Florida if the total prize value meets applicable thresholds. Sponsor will comply with all applicable registration requirements prior to the commencement of the Sweepstakes.

18. WINNER'S LIST

To obtain the name of the winner, send a self-addressed, stamped envelope to: Orchard Church — Korn Guitar Sweepstakes Winners List, 12405 E 120th Ave, Brighton, CO 80640, within [NUMBER] days after the Drawing Date. Winner's name will also be posted at [WEBSITE URL] within [NUMBER] days after October 22, 2026.
